Shared Health leads the planning and coordinates the integration of patient-centered clinical and preventive health services across Manitoba. The organization also delivers some province-wide health services and supports centralized administrative and business functions for Manitoba health organizations.

 

 

Position Overview

  • The Renal Nurse Navigator is a health care professional with specialized knowledge of renal disease who provides guidance, support, and self-management capacity to patients and families with chronic kidney disease and transitioning to end stage renal disease and coordinating seamless transitions of patients moving thought the Regional Chronic Kidney Disease Program.
  • The Renal Nurse Navigator assists with identifying and helping to overcome health system and individual patient level barriers to care, provide health education, work to increase self-management skills, improve coordination of care, facilitate communication among members of the health care team, and provide psychosocial support through their journey.
  • The Nurse Navigator will foster collaboration and knowledge sharing with the care team in order to provide informational continuity and person-centred care.

Experience

  • Four years' experience within the last five (5) years in Renal Health.
  • Knowledge with dialysis modalities including Hemodialysis (HD), Peritoneal Dialysis (PD), Home Hemodialysis (HHD), Renal Transplant, and Conservative/Palliative Care.

Education (Degree/Diploma/Certificate)

  • Baccalaureate of Nursing required.
  • Master’s degree preferred.
  • Completion of Manitoba Nephrology Nursing Course Required (or equivalent).
  • Completion of a Nephrology Related program preferred (Canadian Nurses Association.
  • Certification – Nephrology (CNEPH).
  • Completion of Manitoba Indigenous Cultural Safety Training (MISCT) required.
  • Completion of a recognized Patient Navigation course or willingness to complete within 12 months of taking the position is required.

Certification/Licensure/Registration

  • Must be registered or eligible for registration with the College of Nurses of Manitoba (C.R.N.M.).
  • This position is subject to a Criminal Records Check and Child Abuse Registry Check.

Qualifications and Skills

  • Demonstrated evidence-based knowledge of Renal disease process, treatments, side effects and complications.
  • Advanced level of communication, facilitation, and presentation skills.
  • Experience in adult education principles.
  • Demonstrated competence in team building, conflict management, and interpersonal effectiveness.
  • Leadership skills and ability to work within a multidisciplinary team setting.
  • Effective verbal and written communication skills.
  • Ability to respect a culturally diverse population through knowledge of a broad spectrum of cultural values, beliefs, and health issues.
  • Working knowledge of computers required.
  • Up to date immunization record.
  • Ability to work with minimal supervision and in multiple environments.
  • Demonstrated proficiency in the use of Microsoft Office (Outlook, Word, Excel, Access and PowerPoint).
  • Develop and maintain data base and reports.
